Application Form

To obtain your LISS/CSCS card you will need to:

1. Attend and pass the BALI ROLO (Register of Landbased Operatives) Health and Safety Awareness Course (see the ROLO pages on this website for details of training providers and courses)
2. Pass the CSCS Health and Safety ‘Touch Screen’ Test, which costs £17.50 to sit. For more information or to book a test, visit www.cscs.uk.com/health-and-safety-test or call 0344 994 4488. N.B. Ensure you book the right test for the card for which you are applying: For green, red, blue and yellow cards it’s the Operative test; for gold it’s the Supervisor level test; and for black and white/yellow it’s the Managerial and Professional (MAP) test.
3. Download a PDF of the LISS/CSCS application form on this web page.
4. Complete the form and include a passport size and quality photograph of yourself and a cheque for £38 for the card, made payable to BALI. Payment can also be made using a debit or credit card by calling 02476 690333. Check that all the required signatures and paperwork are also included and return your application to: LISS/CSCS Applications, BALI, Landscape House, Stoneleigh Park, Nr Kenilworth, Warwickshire CV8 2LG.

Your card will be sent to the address indicated on your application form within 28 days, subject to all the required paperwork being included and completed correctly. If anything on your paperwork is incorrect, or something is missing, your application will be returned to you for correction. This will inevitably delay despatch of your card.
